Important Registration Information

Head Start and Early Head Start program teams, representing education, family service, and health, should register as soon as possible (maximum 4 staff per program).  Space is limited to 50 participants

• This is a train-the-trainer model and is appropriate for:

  • Head Start/EHS teachers, family advocates, and home visitors who work directly with families

  • Center/site managers, health/nutrition coordinators, education managers/specialists who will support use of the curriculum in your program, and who have the capacity to train other staff

• We highly recommend that you invite a community partner to join you who is committed to oral health education and has the capacity to train, such as a health/dental educator, nurse, public health dental hygienist, dentist, etc.  Please ask your community partner to register themselves online.

• Registration fee of $25.00 per person covers all materials, supplies, books, and food/beverages. You may pay online using a credit card.  If you wish to pay by check, please make checks payable to "PHSA" and mail to 3700 Vartan Way, Harrisburg, PA, 17110.

EXPECTATIONS:

1. We expect your team to do at least 1 training for your program within 6 months of the training.

2. We expect your community partner to help with your 1^st training, and to continue to provide more training in the future to sustain this effort with your program staff and families.

NOTE:  A pre-training survey monkey will be emailed to all program teams who are registered. You must complete the survey prior to the training, as this is part of the required evaluation plan for the DentaQuest Grant.  One person from each program team, e.g. your designated “Point of Contact”, must complete this survey monkey prior to the training event.  Completion of the survey is required for your program to be able to participate in the training.
